Association of socioeconomic status with myocardial infarction and complications of the myocardial infarction

Aim: To determine the association socioeconomic status with myocardial infarction and complications of the myocardial infarction.
Material and Method: We studied 67 patients (40 men and 27 women) who were admitted to Internal Medicine Clinic of Haseki Education and Research Hospital in between June 2007 and July 2008.
Results: Of the patients, 40 (59,7 %) were men and 27 patients (40,3 %) were women. The average age of the patients was 58,6±9,77 years, ranging between 30 and 85. It was found that there was a statistically significant difference between patients’ health insurance types (p=0,001). The rate of smoking men was higher than smoking women. So, smoking according to gender was also found statistically significant (p<0.05). Hypertension was seen in men more than women which was statistically significant according to sex (p=0,005). There was no association between complications seen after myocardial infarction and patient’s health insurance types (p>0,05).
Conclusion: It was found that there was a statistically significant difference between patients social security institute (p=0,001). Between gender and smoking rates are seen as statistically significant difference (p<0,05). Higher rate of smoking of men than women were found. The incidence of hypertension was found statistically significant relationship between gender (p=0,005). The incidence is significantly higher in male patients. Of the patients developed myocardial infarction complications as Yeşil Kart rate of 32.6 %, SSK 54,3 %, Emekli Sandığı 6,5 %, Bag-Kur 4,3 %, without social security 2,2 %. It was found that there was a statistically significant difference between the social security institute and complications after myocardial infarction.
Conclusion: We conclude that low socioeconomic status (low income) and working in a stresful job may be precipitating factors in the occurance of MI and MI complications.

The efficency of single dose methotrexate treatment with ectopic pregnancy |

Objective: The efficiency of single dose methotrexate treatment applied on patients with ectopic pregnancy is researched.
Materials and method: 30 patients in between ages of 21 and 40 who have received single dose MTX treatment due to ectopic pregnancy is included in the study. All the patients were stabile as hemodynamically. The serum B-hCG levels, USG findings prior to the MTX treatment are recorded. Serum B-hCG levels are monitored in accordance with the single dose MTX protocol.
Results: The average age of the 30 cases taken in the study is 30.63±4.36. The B-hCG levels of the cases as changing in between 160 and 10910, their average is 2386.80±2498.68. The MTX doses they use as changing in between 50 mg and 100 mg, its average is 67.67±13.50. While 18 (60 %) of the cases had no previous surgical history. While the mass size of 6 (20 %) of the cases is below 4 cm, in 24 (80 %) of cases it is between 5 cm and 5.5 cm. 4 (13.3 %) of the cases is receiving additional dose of MTX. Laparotomy is applied on 3 (10 %) of the cases due to tubal rupture developing during the treatment.
Conclusion: The medical treatment to be applied on non-ruptured ectopic pregnancies has many advantages. Less tubal damage occurs by the medical treatment, it decreases the morbidity for anesthesia and surgery.

Communication skills of students of Amasya medical vocational school |

Introduction: This search was carried out what the senses of communication skills of midwifery and nursing profession students of Amasya Medical Vocationel School are. Whether the senses of communication skills created a noteworthy difference according to some variancer was also searched.
Materials and Methods: The sample of the search was composed of 232 students. The data related to the search of serves of communication skills was gothered by “Personel Informa-tion Form” which was prepored by Korkut to find out the demographic features of the students and “The Scale of Evaluation of Communication Skills” which was prepared by the researcher (1997) and then whose volidity and reliability was proved on college student. The data was evoluated by SPSS 11.5 statistics programme. In the analysis of the data; frequancy, Kruskal-Wallis (KW), Man Whitney U testi, were used. Signifance level was chosen as p<0.05.
Implications: It demonstrated that the senses of communication skills of students of Amasya Medical Vocational School were quite high. (X:101,69)
Conclusion: While here was a noteworthy relationship between the department that is studied and the level of academic succes, it was found out that other variannces did not compose any differences upon the senses of communication skills.

The last developments about the diagnosis and treatment of congenital penile curvature (chordee) |

Penile curvature which is seen in adults was firstly described by Nesbit as possibly congenital1 in 1965. Congenital penile curvature is not a rare anomally and its prevelance is reported between 0.04 and 0.6. But its real prevelance might be much more higher. Because most curvatures have mild degrees and are clinically insignificant. When those patients come to puberty age they start to realise their penil deviation with erection. Diagnosis is with photographie of erectile penis. All of the penile deviations don’t require treatment. Treatment is necessary when coital functions are impaired. The only cure choice is surgery. There might be need for pyschological evaluation and support before treatment. Although there are a lot of techniques for penile corrective procedures none of them is without complications. By the way of compositon of dorsal nevre complex and thickness of tunica albuginea; dorsal midline plication at 12 o’clock side has been capable.

Chylothorax caused by trombosis of the superior vena cava in behcet's disease |

Behcet’s Disease is a systemic disorder that may involve various organs. The prevalence of pulmonary involvement in Behcet’s is 5 % to 10 %. Chylothorax secondary to superior vena caval obstruction is a rare complication and has been reported in only a few cases. We report a 40-year-old male with Behcet’s Disease who developed chylothorax associated with superior vena cava thrombosis.

Treatment alternatives after failure of urethral stent implantation: A case report and review of the literature

Urethral stricture is a benign pathology of urethra. However it can often repeat and performed lower urinary tract symptoms. Urethral stents are one of minimally invasive treatment on persistent urethral strictures. However, recurrent urinary tract infections, stent migration, encrustation and recurrent urethral strictures may ocur after urethral stents. The treatment is controversial when complication ocur. Urethroplasty is highest success treatment method. Complete excision of the obstructed urethra containing the stent with the surrounding periurethral fibrosis is required on permanent therapy particularly. The patients are informed preoperatively about intervention therapies.

Asymmetric bilateral traumatic dislocation of the hip with obturator nerve injury: A case report

Bilateral traumatic dislocation of the hip is not often encountered and even more rare is asymmetric dislocation from high impact trauma, with one hip dislocated anteriorly and the other posteriorly. This case is presented of a 24-year old female, who sustained asymmetric bilateral traumatic dislocation of the hip in a traffic accident when not wearing a seatbelt in the driving seat. The patient was treated conservatively and after 2 years follow-up was evaluated according to Thompson and Epstein clinical and radiological scoring. The clinical and radiological results were excellent.

Pleomorphic adenoma of the palate in the first decade of life

Salivary gland tumors of the oral cavity originating from minor salivary glands are rare. Swelling of the oral mucosa should include pleomorphic adenoma in differential diagnosis. In children sweling of the oral cavity are usually abscess, mucosal cysts and salivary gland tumors. Here in our case an 8 eight year old girl with a swelling of the hard palate is diagnosed as pleomorphic adenoma. Here we aimed to remind the differential diagnosis, diagnosis and treatment of the palatal pleomorphic adenoma with a literature review.
