Yrd. Doç. Dr. Timuçin Alar
Çanakkale Onsekiz Mart Üniversitesi Tıp Fakültesi Göğüs Cerrahisi Anabilim Dalı

SUMMARY

Thoracic outlet syndrome (TOS) is a syndrome that reveals different symptoms resulted from compression of the neurovascular structures in the region between the scalen muscles and the first rib. In order to diagnose accurately, clinical presentation may be evaluated as neurogenic and vascular TOS. A careful and detailed medical history and physical examination with provocative tests are the most important diagnostic tools for proper identification of TOS. Imaging of the chest and the cervical spine, nerve conduction studies can provide helpful information regarding diagnosis. For most patients with TOS, clinical management usually starts with conservative treatment including exercise programs and physical therapy. Surgical intervention is indicated for vascular and true neurogenic TOS and for some patients in whom nonoperative therapies fail.

APPROACH OF THE HEALTH PERSONNEL TO THE WOMEN WHO HAVE BEEN SUBJECTED TO VIOLENCE

Yrd. Doç. Dr. Özlem Erel
Adnan Menderes Üniversitesi Tıp Fakültesi Adli Tıp Anabilim Dalı

SUMMARY

Violence against women affects women’s lives negatively and it makes up a burden on the health care system. In spite of the violence they have been struggling with, 92% of the women who were subjected to physical or sexual violence don’t resort to any public or private institution. Despite the fact that their health is affected they don’t resort to any health facilities which is quite a sad condition. Because most of the help considering violence comes from these institutions. Health care providers, play a key role in eliminating or reducing the effects of violence. Health care institutions are the only institutions in touch with a woman’s life at any stage. Health care providers firstly need to know what kind of questions they should ask to the women and the girls who were subjected to violence, how to act during the process of evaluation and reporting of the findings and then they should know how to help out in legal aid. In our country, together with the recent legislative changes, education programs for various occupational groups have been given about violence against women.

SUMMARY

The risk of developing life threatening bacterial and fungal infections is significantly high in neutropenic patients. In such cases, it is essential to regulate neutropenia, to apply proper and efficient antimicrobial agent and to continue with the other supporting treatments. Donor Granulocyte Transfusion (DGT) aims to provide polymorphonuclear leukocytes in sufficient amount, quality and functionality. Recent advances in early infection detection, development of more efficient antimicrobial agents, and use of recombinant hematopoietic growth factors and infection risks associated with transfusion and HLA alloimmunization as well as life-threatening acute toxicities has limited the DGT utilization. In recent years, DGT has become topic of interest again as a result of increased presence; thus harvesting of granulocytes in circulation, which can be attributed to development of hematopoiteic growth factors. In this paper, we have a look at, indications, clinical usage and technology of granulocyte collection and transfusion.

SUMMARY

Human metabolism is affected by day light in a daily cycle. Therefore, seasonal changes have an impact on the psychological, emotional, physiological, neurochemical and hormonal mechanisms of the people. Seasonality has a potential impact on the nutrition and health status of the society. The major components that are affected by seasonal differences are food preferences, quantity and diversity. The impact of seasonal changes is more perceptible in developing societies. Seasonal climate variations have different effects on people, but it is not easy to determine exactly these effects on nutritional status of individuals. Food availability is the basis of food security. In spite of the facilitator impact of globalization and open market on food availability in the world, approximately one billion people suffer from undernutrition due to lack of food in developing countries. Seasonal variations affect availability of food both physically and economically. Influence degree of the seasonality on energy, food and nutrient intake changes depending on economic status and differences in climate between seasons and countries. Because of limitations of economic and difficulties of food consumption surveys seasons are not taken into account. This situation causes potential errors on food intake according to season. Therefore, it is important to determine the degree of impact of seasonal changes on physiological and biological parameters while conducting a study. The determination of seasonal changes in dietary habits is essential on the assessment of nutritional status. In this article, the impact of seasonality and global factors on food consumption, diversity and nutrition behavior of individuals is examined.

SUMMARY

This study has been planned and performed in a descriptive design to determine nonprescription drug usage among individuals, their health responsibility levels and to examine the relations between them.

The study enrolled 397 subjects selected from 50.000 people getting health service from Yenişehir Health Center in Erzurum. This number of participants is based on The Minimum Sample Size, developed by Yamane at an alpha significance level of 0.05 and adapted into Turkish by İdil. These 397 people were divided up into 7 neighborhoods so the sampling size from each location was calculated to be 59 participants. However to avoid the inconvenience of fractional numbers in calculations, the number needed for sampling was raised to 60 participants.for each location. Then, one individual from each randomly chosen house was included in the study and the questionnaire was applied until the number of subjects reached to 420. Later because of erroneous responses and incorrect data, nine of the questionnaire forms were eliminated and 411 of them were evaluated.

The questionnaire form included questions about the socio-demographic features, the health responsibility of the persons, their over-the-counter drug (OCD) usage. Data were analyzed by correlation, one-way variance analysis (ANOVA), and chi-square test, Student t-test and percentage distribution.

In the present study, it was determined that health responsibility levels of the participants were low, and the descriptive features such as age, education, marital status, the places where they spent most of their lives and their professions all affected their health responsibility level. It was determined that 68% of persons used OCD drugs which consisted mostly of analgesics and antipyretics It was determined that educational status was the most important factor affecting drug usage, and sex, age, marital status, absence or presence of a chronic disease and also income status had also important effects on OCD drug usage

Consequently, it was determined that there exists important correlations among OCD drug usage, health responsibility level and the socio-demographic features of persons.

SYMPATHECTOMY

Yrd. Doç. Dr. Ayşe Tuç Yücel
Celal Bayar Üniversitesi Sağlık Hizmetleri Meslek Yüksek Okulu

SUMMARY

The role of the sympathetic nervous system in medicine can be understood by inducing changes in degeneration of sympathetic neural pathways. Several methods of inducing peripheral lesions such as the surgical removal, neural growth factor depletion, auto-immune and chemical destruction to novel approaches by immunotoxins and transgenic animals were used to perform lesions. Surgical ablation can be performed by open removal or electrocoagulation of the sympathetic chain, or minimally invasive procedures using stereotactic thermal or laser interruption. Chemical sympathectomies use alcohol or phenol injections to destroy temporarily the sympathetic chain. It is commonly performed for palmar or plantar hyperhidrosis, Buerger's disease and critical lower limb ischemia. Neuropathic pain is defined as pain initiated or caused by a primary lesion or dysfunction in the nervous system. Surgical and chemical sympatholyses may help to prevent it. The practice of chemical sympathectomy is still under investigation because of complications which may be important. Therefore, more experimental studies and clinical trials of sympathectomy are required to establish the overall effectiveness and potential risks of this procedure.

SUMMARY

One of the most effective interventions that may lead to permanent changes in the life-style of individuals and effect their life-quality is the opening of an intestinal stoma. Permanent stoma’s may remain for a lifetime period. The most common reason that may require an opening of an abdominal stoma are colorectal cancers. In our country, the clinical specifications of colon cancer and studies carried out in surgical centers demonstrate that the number of cases have dramatically increased. The objective in the care of stoma is to improve the life quality of the patient and keep it at the most highest lever as possible. Appropriate nutrition, may accelerate the recovery period after a colorectal and stoma surgery. The eating options or special diets of patients with a cytoma must be determined according to the type of the stoma. For this purpose, the healthcare staff plays a fundamental role to determine the nutritional strategy of these patients while the type of nutrition must be adjusted according to the life style of the stoma patient. Investigations on various databases demonstrated that there were a limited number of studies carried out related with the nutrition and food selection of patients with an opened stoma. A correct diet information can be life-saving in patients with a new stoma opening. For this purpose, we need a guideline manual which may provide miscellaneous information related with the nutrition of stoma patients and the method that may be applied to solve their nutritional problems.

SUMMARY

The general pattern of human body shape development is a species-specific characteristic. Historical artwork, sculpture and anatomical drawings show fundamental commonalities in the depiction of body shape of late term fetuses, newborns and infants. Discrete populations of living humans, however, present a diversity of body sizes and shapes. Posture and movement are regulated by a number of structures and functions within and without the nervous system. Alterations of movement and posture lead to imbalance and, thus, cause serious public health problem. Despite the importance of good postural adjustments to the safe and eficient performance of movement, little is known about the properties of these postural accompaniments. The purpose of this article is to provide a review of properties of postural adjusments that accompany a variety of limb, trunk and others movements and propose a schema for the coordination of good posture and abnormalities.
